Job description
Martin Luther King, Jr. Community Hospital is seeking a Case Manager to support physicians and an interdisciplinary team in coordinating patient care, improving outcomes and satisfaction while managing costs.
The role encompasses utilization management, care progression and discharge planning across the hospitalization continuum. The ideal candidate will have a BSN, California licensure, BLS certification, and preference for case management certification.
