Job description
LCMC Health in Metairie, LA seeks a Care Manager to coordinate patient care across the continuum. You will develop individualized care plans, conduct outbound assessments, and collaborate with the care team to optimize outcomes. Shift schedule includes 12-hour days with weekend rotation.
Requirements include an active Louisiana RN license and at least 2 years in care management or ED nursing. You will document workflows in EPIC and support discharge planning and patient education.
