Job description
Frederick Health is seeking a registered nurse to serve as Care Manager, coordinating care across hospital and ambulatory sites. You will assess patients, plan and implement care, address social needs, and guide families through the health system.
In this role, you will collaborate with physicians and a multidisciplinary team to improve outcomes, manage transitions, and promote evidence-based care while complying with FH policies and standards.
