Job description
Orlando Health Melbourne Hospital in Melbourne, FL is seeking an experienced RN Care Manager to lead patient care coordination from admission to discharge. You will collaborate with physicians, nurses, and social services to ensure appropriate levels of care and smooth transitions for patients.
The role requires a Florida RN license, at least 3 years in care management or related fields, and Handle with Care certification for Behavioral Health.
