Job description
Position:
RN Care Manager with a focus on patient assessment, care planning, and coordination.
Responsibilities:
Conduct comprehensive patient assessments
develop personalized care plans
address barriers to care
facilitate communication among patients, families, and healthcare providers
monitor patient progress
coordinate referrals and transitions of care
educate patients on self-management
maintain accurate health records
participate in quality improvement initiatives
Qualifications & Skills:
Bachelor’s or Master’s Degree in Nursing
minimum 2 years of clinical nursing experience
current RN license in Illinois
CPR/BLS certification
preferred CCM certification
Key Duties:
Collaborate with interdisciplinary teams
ensure regulatory compliance
contribute to enhancing patient outcomes
Compensation & Benefits:
This role involves weekend shifts, paying between $40.45 and $62.70 per hour, with benefits including medical options, tuition reimbursement, retirement plans, paid time off, and career growth opportunities.
