Job description
TEEMA is seeking an Registered Nurse (RN) Care Manager in New Jersey to provide patient-centered care across clinics, centers, and patient homes. The role involves assessing needs, coordinating services, implementing care plans, and evaluating outcomes to ensure high-quality care.
The on-site position may require travel to patient homes; on-call duties and collaboration with a multidisciplinary team are essential. A BSN is preferred, with RN licensure in NJ and BLS certification within 90 days.
