Job description
Visiting Nurse Health System Inc. in Rome, GA is seeking a full-time Care Manager RN to assess, coordinate, and monitor community-based services for elderly or disabled clients under CCSP.
You will perform home visits, reassess needs, and collaborate with physicians, social workers, and caregivers to ensure continuity of care. You will lead in-home assessments, develop care plans, and document progress while coordinating transitions from hospital to home.
