Job description
Molina Healthcare in Jackson, MS, is seeking a Registered Nurse care manager to coordinate member care across the delivery continuum. You will collaborate with the care team to advance outcomes and support cost-effective services.
You will perform comprehensive assessments, develop care plans, conduct visits, monitor progress, and promote integrated services across behavioral health, LTSS, and community resources. Travel will be required as needed.
