Job description

Laredo Medical Center is seeking an RN Care Manager to coordinate discharge planning, transitions of care, and case management to ensure optimal patient outcomes.

This role requires collaborating with interdisciplinary teams, reviewing medical records for appropriateness, and maintaining compliance with federal, state, and accreditation standards. RN licensure is required; ACM/ CCM credentials are preferred.
