Job description
Charlotte Hungerford Hospital is a 122-bed acute care community hospital in Torrington, CT. The Case Management team works to ensure optimal health, access to care, and appropriate utilization of resources, coordinating with nurses, physicians and health plans to discharge patients thoughtfully.
The RN in this role uses the nursing process to assess and facilitate the most appropriate level of care, ensuring patient satisfaction, confidentiality, and culturally sensitive care.
