Job description
Charlotte Hungerford Hospital in Torrington, CT, invites experienced registered nurses to join our Care Management team. This role focuses on coordinated patient care, discharge planning, and collaboration with physicians, nurses, social workers, and health plans to ensure optimal outcomes.
Qualified applicants hold an active Connecticut RN license, a Bachelor’s degree in Nursing, at least 3 years of acute care hospital experience, and case management certification (ACM/CCM) preferred.
