Job Title : RN, Care Manager (Home Health)
Pay rate:
Weekly Gross: $
(Weekly Stipend non-taxable: $
Taxable Weekly:$800)
Blended Hourly: $70.00
Shift:
Days 8:00 AM to 4:30 PM
40 hours/Week
Provides excellent patient care by assisting in collaboration, development, implementation, revision and reporting of the case management program.
Acts as a liaison between the patient, family, nurse, physicians, multidisciplinary team and patient's healthcare benefactor to optimize outcomes.
Serves as a consultant to the healthcare team on specific patient items.
Day Shift,40 hours per week , coverage area is Lake County, CA ; mileage (government rate) is in addition as travel is needed to see patients
THIS IS FOR HOME HEALTH NOT HOSPITAL, Home Health experience is A MUST/REQUIRED.
MUST have HCHB software experience is required
Education and Work Experience: Bachelor's Degree in Nursing (BSN): Preferred
Experience in a care management role: Preferred
Licenses/Certifications: Registered Nurse (RN) licensure in the state of practice: Required
Case management certification: Preferred
Basic Life Support (BLS OR HS-BLS OR RQI BLS) certification: Required
