We're hiring an RN Care Manager (Home Health) for a 16-week travel contract in beautiful Simi Valley, CA .
Location: Simi Valley, CA
Start Date: 08/17/2026
End Date: 12/06/2026
Schedule: Monday–Friday | 8:00 AM–5:00 PM (40 hrs/week)

Recent Home Health experience
Homecare Homebase/PointCare experience required
Epic experience required
Valid Driver's License, reliable vehicle & current auto insurance
BLS Certification
