Job description
HomeWell Care Services in Canton, GA seeks a compassionate RN Care Manager for a part-time, hybrid role. This position focuses on in-home assessments, care planning, and client-family education while supporting caregivers and coordinating with the healthcare team.
Newly licensed RNs are encouraged to apply. Travel within Canton and Cherokee County is required; some administrative work may be done remotely. Training and mentorship are provided to grow clinical and consultative skills.
