Job description

Arizona Priority Care in Chandler, AZ is recruiting an RN Care Manager to lead telephonic and in-person care coordination for Medicare Advantage members. You will assess needs, support discharge planning, and collaborate with clinical teams to ensure effective transitions of care.

The role requires an active AZ Nursing license with BSN preferred and at least 5 years in acute care or care management. After a 60-day in-office period, hybrid options may be available.
