Job description
Mercy is seeking a Care Manager to support discharge planning from admission to discharge in collaboration with the healthcare team. The role may involve addressing complex psychosocial needs and aligning with Mercy's mission and service standards.
Ideal candidates have 2-3 years in acute care, nursing licensure in Missouri, and BSN; BLS required at hire or within 90 days; Case Management certification preferred.
