Job description
The RN Care Manager II – IPCM collaborates with an interdisciplinary team to assess, plan, advocate, and coordinate patient care from admission to discharge, ensuring safe and effective transitions.
Responsibilities include evaluating patients’ health needs, developing personalized care plans, and coordinating services to improve health outcomes.
Effective communication with patients, families, and healthcare providers is essential, along with advocating for patient needs and promoting health education.
Clinical expertise, critical thinking, and compassion are vital to support patient well-being and enhance healthcare results.
Qualifications include a nursing degree, RN licensure, and experience in acute care; certifications in BLS and case management are preferred.
