Job description
Horizon Blue Cross Blue Shield of New Jersey is seeking an experienced RN to manage care for high-risk members within our Medicare/Marketplace programs. The role focuses on moving members through the continuum of care with a member-centric plan of care, and mentoring staff on best practices in care coordination.
The caregiver will support preventative care opportunities, perform routine case management duties, and engage with external providers to ensure optimal outcomes across the care
