Job description
Lakeland Care, Inc. is looking for an RN Care Manager to join an interdisciplinary team serving frail elders and adults with disabilities. This float position, primarily within Dodge County, involves arranging care services based on comprehensive assessments and coordinating care transitions. Applicants must have a current RN license in Wisconsin and a preferred bachelor's degree in nursing, along with two years of skilled nursing experience. Travel may be required within a 2-hour radius of the service region.
