Job description
Molina Healthcare is seeking an RN care manager based in Minooka, IL to support care management and LTSS activities. You will collaborate with a multidisciplinary team to coordinate member care across the continuum and ensure progress toward outcomes.
Responsibilities include completing assessments, coordinating waiver enrollment, developing and monitoring care plans, and promoting integrated services while navigating regulatory timelines. Travel up to 25-40% may be required.
