Job description
PACE North is seeking an RN Care Manager to join our team in Traverse City, Michigan. You will be a decision maker, advocate, and partner in care for adults 55+ who qualify for nursing facility services, helping them live safely at home.
As part of an interdisciplinary team, you’ll assess health needs, coordinate services, and support transitions from hospital to home, while upholding our core values of quality, safety, compassion, integrity, collaboration, and community.
