Job description
Molina Healthcare is seeking an RN Care Manager in Illinois to support LTSS care coordination and collaborate with a multidisciplinary team. You will help ensure member progress toward outcomes and contribute to cost-effective, high-quality care across the continuum.
Responsibilities include care plan development, ongoing monitoring, waiver service coordination, and assessing medical necessity. Travel of 25-40% may be required depending on state/contractual needs.
