Molina Healthcare is seeking an experienced Care Manager (RN) to coordinate long-term services and supports (LTSS) and improve member outcomes through integrated care delivery in Trempealeau, Wisconsin.
Primary duties include comprehensive assessments, waiver enrollment/disenrollment, care plan development with members and providers, ongoing monitoring, and promoting service integration, with 25–40% local travel.
