Job description
Molina Healthcare is seeking a Care Manager RN in Illinois to coordinate long-term services and supports (LTSS) and waiver services. You will collaborate with an interdisciplinary team to ensure member progress toward outcomes, perform home visits, and guide members and caregivers through enrollment and service plans.
Responsibilities include assessing medical necessity, developing and monitoring care plans, and facilitating access to community resources across settings.
