Job description
Piedmont Health Services, Inc. is seeking an RN Care Manager for the PACE program in Burlington, NC. The role involves direct and indirect nursing care, chronic disease management, and collaboration with an interdisciplinary team.
Expect after-hours on a rotational basis for urgent home visits, with a schedule Monday through Friday, 8:00am–5:00pm, located at 1214 Vaughn Rd, Burlington. Licensure in NC required; CMS competencies expected.
