Job description
Centene is seeking a registered nurse care manager to develop, assess, and facilitate complex care management activities for primarily physical health needs of members. You will create personalized care plans, educate families, and coordinate with providers to address unmet needs.
The role requires up to 75% local travel in Wayne and Macomb counties, with the option to work remotely from home the remaining time. Mon–Fri, 8am–5pm; mileage reimbursement applies.
