Leading MLTC in the NY area is seeking experienced REMOTE Care Managers
CANDIDATES MUST RESIDE IN THE METROPOLITAN NEW YORK ARE
Responsibilities
Perform assessments of members’ medical, psychosocial, physical and spiritual nee
ds.Develops, implements, and monitors person centered service pl
an.Ensures continuity of care across all healthcare settings by collaborating with the interdisciplinary care team, PCP, member/member’s representatives, and specialis
ts.Utilizes benefit structure and community-based resources to meet member’s needs
Educates members on disease processes, preventative health, and quality of life intervention Completes initial telephonic assessments and reviews UAS -NY visit data as assigned by the Supervisor of Coordinated Care in accordance with departmental workflows
Background
RN, State of NY
Previous Care Management experience in an MLTC or Managed Care Setting
Ability to attend monthly meetings in Manhattan
