Job description
BoldAge PACE is dedicated to helping older adults live meaningful, independent lives. The RN Care Manager will assess, plan, and coordinate nursing care across clinic, center, and participants’ homes in New Jersey.
You will collaborate with the IDT, delegate tasks to aides, ensure follow-up, and educate participants and families on self-care and safety.
This role requires NJ RN license; BSN preferred; BLS within 90 days. Eager to mentor and improve home-based care.
