Job description
WellMed, part of the Optum family, is seeking a Healthcare Manager in Texas to support patients with high-risk conditions and help them navigate the healthcare system. You will educate, empower, and advocate for patients and families to improve quality of life.
You will collaborate with physicians, nurses, pharmacists, social workers, and educators; document in EMR; participate in PCC meetings and weekly reviews; and help transition patients from inpatient to outpatient settings while pursuing
