Westchester Institute for Human Development (WIHD) in Valhalla, NY invites an experienced Registered Nurse to join as Care Coordinator & Clinic Floor Nurse. This full‑time role blends direct clinical nursing with care coordination and chronic care management to improve patient outcomes.
Youll work closely with the Nurse Manager, providers, and staff to optimize clinic operations, patient experiences, and continuity of care while supporting transitions and education across the care team.
