Job description
Molina Healthcare is seeking a Registered Nurse Care Manager to support LTSS navigation and care coordination across settings. You will collaborate with a multidisciplinary team to ensure high-need members progress toward outcomes and that services are integrated, cost-effective, and patient-centered.
Responsibilities include assessments, waiver management, care plan development and monitoring, and leading ICT meetings. Travel up to 40% may be required based on state contracts.
