Title: (Registered Nurse- Case Manager ) – Contract
Location: Syracuse, NY
Job Details:
Local Pay Rate- $66hr
Travel Pay Rate- $40hr + $1414 weekly stipend
40 hours Guaranteed
5 shifts x 8hr shifts
Day Shift
13-week assignment
Description of job: The Case Manager at SUNY Upstate Hospital is responsible for coordinating patient care plans, ensuring timely and efficient transitions through the continuum of care. This role involves collaborating with medical teams, patients, and families to optimize healthcare outcomes and manage resource utilization in compliance with hospital policies and regulatory guidelines.
Qualifications:
Current New York State professional license/registration
Certified in Basic Life Support (BLS) and able to be recertified
Minimum 1.5 years of recent experience (within the past 2 years) in an acute care facility of similar size and level
Travelers must have experience in hospitals of equal or greater size, or trauma centers
Experience with EPIC Electronic Medical Records system preferred
Excellent interpersonal and communication skills
Responsibilities:
Coordinate and monitor comprehensive patient care plans across the continuum of care
Collaborate with interdisciplinary teams to ensure timely and appropriate discharge planning
Assess patients’ medical, psychosocial, and financial needs to support individualized care
Facilitate communication between patients, families, and healthcare providers
Ensure compliance with hospital policies, regulatory requirements, and payer guidelines
Document case management activities accurately in the electronic medical record (e.g., EPIC)
