Job description
The Staff Pad partners with a healthcare organization in Hamilton, TX to recruit an experienced Case Management Manager. You will lead the Case Management team to coordinate patient care from admission through discharge, collaborate with physicians and nursing staff, and remove barriers to timely discharge while promoting safe, cost-effective care.
