Job description
Saint Francis Hospital in Hartford, CT, is seeking an experienced RN Case Manager to coordinate patient care, assess needs, and develop care plans in collaboration with the care team. You will monitor progress and advocate for patients to ensure smooth transitions between care stages.
The role requires graduation from an accredited nursing school and a valid CT RN license; a BSN is preferred. The position is a 40-hour work week on a 12-hour day shift within a Catholic health system.
