Job description
RN.com is seeking an experienced RN Case Manager to coordinate and oversee patients' individualized care plans within a hospital setting in Colorado Springs. You will collaborate with physicians, therapists, nurses, patients, and families to facilitate rehabilitation goals and ensure safe transitions of care.
Required qualifications include an active CO/compact RN license, BLS certification, and at least one year of recent case management experience in an acute care hospital.
