Job description
Doctors HealthCare Plans, Inc. is seeking a Nurse Case Manager to coordinate the continuum of care for assigned patients, ensuring optimal resource use and compliance with medical regimes.
The role requires RN/LPN licensure, preferred managed care experience, and bilingual English/Spanish abilities. The Nurse Case Manager will participate in case conferences, collaborate with physicians, document and review care plans, and advocate for patient access to services.
