Job description
Bon Secours Charity Health System is seeking a Case Manager (RN) to oversee day-to-day operations of the Case Management programs, ensuring appropriate utilization of hospital resources while maintaining high-quality patient care.
In this role you will lead patient assessments, coordinate with staff and physicians, determine admission criteria, monitor care plans, mentor staff, and perform related duties to support efficiency and outcomes.
