Job description
Lee Health in Fort Myers, FL seeks a Case Manager with clinical expertise to facilitate discharge planning and manage utilization and quality of care. You will be part of a multidisciplinary team, guiding patients across the care continuum and linking them to needed resources and services to maximize functioning.
Requirements include an ADN in nursing with ongoing pursuit of a BSN, at least 3 years of clinical experience, and an active Florida RN license.
