Job description
Singing River Health System in Gulfport is seeking a Case Manager focused on clinical quality, resource management, and coordinating patient care across ages within the SRHS system.
You will participate in daily discharge planning meetings, address barriers to discharge, and foster seamless communication between inpatient and outpatient care teams while upholding SRHS policies and standards.
