Job description
RN.com in Portland, ME is seeking an RN Case Manager to coordinate patient care from admission through discharge, working with physicians, nurses, and external providers to ensure safe transitions. The assignment lasts 13 weeks and starts ASAP.
Eligible candidates hold a ME/compact RN license, BLS, and have 2+ years of acute hospital case management experience, with discharge planning expertise and familiarity with Epic.
