Job description
Memorial Healthcare System in Pembroke Pines, FL, is seeking an RN Case Manager to coordinate care and service for patient populations from admission through discharge, partnering with interdisciplinary teams to develop a comprehensive care plan and ensure efficient transitions.
You will conduct in-depth assessments, facilitate rounds, monitor LOS, and address clinical, psychosocial, and financial needs to support safe progression of care.
