Job description
Advocate Health in Wisconsin seeks an experienced Registered Nurse for home health case management. You will assess patients, establish plans of care per physician orders, and provide direct nursing care while adjusting the plan based on patient needs and responses.
You will document assessments and care activities, coordinate with physicians and family, and participate in discharge planning and quality improvement.
