Job description
Henry Ford Health in Detroit, MI is seeking an experienced Case Manager-RN to coordinate patient care across settings, ensuring smooth transitions and efficient resource use.
Requires active RN licensure and 5+ years in case management; strong clinical judgment, communication, and EHR proficiency. You will work with the care team on discharge planning, quality improvement, and interdisciplinary collaboration.
