Job description
Methodist Le Bonheur Healthcare in Memphis seeks a Case Manager III to coordinate care and service delivery for a designated patient group, fostering collaboration among patients, families, physicians, and the health care team. You will assess, implement, coordinate, monitor and evaluate services and resources to optimize outcomes and cost effectiveness.
