Job description
Methodist Hospital Northeast is seeking a dedicated Case Manager to support patients and families through every step of their care journey. You will coordinate services, connect resources, and develop individualized care plans in partnership with physicians, nurses, and department leaders to ensure safe transitions and high-quality care.
