Job description

Progeny Human Resource Solutions seeks a Full-Time Registered Nurse Case Manager in Tallahassee, FL. The RN CM Care Coordinator will facilitate the interdisciplinary plan of care, evaluate clinical care, medical necessity, admission status, level of care, and resource management.

You will coordinate activities that promote quality outcomes and patient throughput while balancing optimal care with appropriate resource use, and identify barriers to throughput to facilitate discharge plans.
