Job description
HCA Houston Healthcare Clear Lake is seeking a Registered Nurse Case Manager to guide patients and families through care transitions. You will coordinate services, develop individualized plans, and partner with physicians, nurses, and case managers to ensure safe, efficient discharge planning and continued recovery.
