Job description
RN.com is seeking a skilled RN Case Manager in New York to support safe discharge transitions and coordinate post-acute services. The role involves identifying high-risk patients, collaborating with the interdisciplinary team, and documenting care in Epic to ensure smooth care transitions.
Ideal candidates have NYS RN license, PRI Certification, and 2 years of recent case management experience in an acute hospital setting. 8-week assignment with 37.5 hours per week. Epic experience is preferred.
