Job description
Luminis Health is seeking a Case Manager to coordinate care for patients, ensuring safe and timely transitions across the continuum. You will assess, plan, implement and evaluate care using a collaborative process, applying guidelines to determine initial and ongoing levels of care and submitting reviews to insurers as needed.
Requirements include a Maryland RN license and 3+ years in a clinical setting; BSN/ADN with equivalence and Epic proficiency are preferred.
