Job description
Mercy Health St. Vincent Medical Center in Toledo, OH is seeking an experienced RN Case Manager to oversee transitions of care and secure payer approvals for services.
You will ensure proper documentation supports patient status, level of care, and DRG classification, collaborating with the care team to optimize outcomes. The role requires a current Ohio RN license, BSN preferred or obtained within 3 years, and at least 3 years of clinical experience with case management certification preferred.
